Step 4

A licensed physician conducts a comprehensive review of:

• your request;

• medical records;

• specialist reports;

• diagnostic imaging;

Step 5

Following review, one of the following may occur:

• Your medical report is prepared.

• Additional documentation is requested.

• A medical consultation or physical examination is required.

• Additional physician review time is required because of the complexity of the request.

• The physician determines that the requested report cannot be prepared.

 

Step 6

Your completed report is securely delivered through the patient portal.

The fee depends on:

• Complexity of the request

• Volume of medical records

• Number of medical conditions involved

• Number of specialist reports reviewed

• Length of the report required

• Estimated physician preparation time

 

Most reports range between $495 and $995 CAD.

If the physician determines that additional review time is required beyond the initial estimate, this will be discussed with you before any further work is undertaken.

Detailed Medical Reports require physicians to provide objective, accurate, and professionally supported medical opinions based on the available clinical information.

• additional medical records are required;

• further physician review time is necessary because of the complexity of the request; or

• the requested report cannot be prepared because there is insufficient clinical information to support the requested opinions.
