Important Information
Detailed Medical Reports require physicians to provide objective, accurate, and professionally supported medical opinions based on the available clinical information.
Our physicians are professionally and ethically responsible for ensuring that every report accurately reflects the medical evidence available at the time of preparation.
Following review, the physician may determine that:
-
the report can be prepared;
-
additional medical records are required;
-
additional specialist reports are necessary;
-
a medical consultation or physical examination is required;
-
further physician review time is necessary because of the complexity of the request; or
-
the requested report cannot be prepared because there is insufficient clinical information to support the requested opinions.
Submission of a request does not guarantee preparation of a medical report or support for any legal, insurance, employment, educational, or governmental application.
Physicians provide independent medical opinions based solely on the available clinical information and accepted medical standards.
Frequently Asked Questions
What is a Detailed Medical Report?
A Detailed Medical Report is a comprehensive written report prepared by a physician after reviewing your medical records and other relevant clinical information. It is substantially more detailed than a standard medical form or certificate.

Often, yes.
Many Detailed Medical Reports require a physician consultation or physical examination to ensure the report accurately reflects your current medical condition.
If an appointment is required, we'll explain the next steps before proceeding.
How long does it take to prepare a report?
Preparation time depends on the complexity of the request and the amount of documentation requiring review.
If additional information is required, we'll notify you promptly through the patient portal.
Can you prepare reports for legal matters?
Our physicians may prepare objective medical reports where clinically appropriate.
However, they do not act as advocates for either party and will only provide opinions that are supported by the available clinical information.
