Choose your service, pay the first installment for your Medical Review Fee, then securely upload your documents using the secure link we send you.
Use the secure portal link that will be provided in your email to complete your intake form and safely upload your supporting documents. Once your intake form is completed, your submitted information will be reviewed. If the applicable Medical Review Fee is higher than the initial payment received, the remaining balance will be charged to the card provided.

The physician will review your request based on the service level selected. If additional information is required, they may call you directly. If the physician determines that a consultation is necessary, they may arrange a convenient virtual or in-person appointment to obtain any additional information needed before completing your documentation.

Form5.ca coordinates eligible uninsured medical-form services across Atlantic Canada. Eligibility depends on your request and your physical location when a physician appointment takes place.
Can I speak with a doctor before submitting my form?
Yes. If you are unsure where to start, you can book a Medical Forms Guidance Consultation with Dr. Janine Karpakis to discuss your form requirements, the information that may be needed and your next steps.
Appointments are available Monday to Thursday, between 4:30 p.m. and 8:30 p.m. Atlantic Time, subject to availability.
This paid guidance appointment does not include an insured medical assessment or form completion.
How do I start if I already have my form?
Choose Standard or Priority Review and complete your Form Review Application payment through Stripe. We will then email you instructions for completing your secure intake and submitting your documents.
How quickly will my request be reviewed?
Review timing depends on the processing option selected and receipt of the required information. Check the current service details before paying.
Form completion may take longer if additional records, clarification or a separate assessment are needed. Priority review does not guarantee completion by your deadline.
Will I need an appointment?
Not always. Some forms can be completed using sufficient existing records and supporting documentation.
Other requests may require a separate virtual or in-person assessment. Any additional work and fees will be explained before you agree to proceed.

No. The physician must have sufficient information and be able to support the documentation medically. Payment does not guarantee a signature or approval of an application or claim.
