• A medical consultation or examination is required.

• Additional physician time is required due to the complexity of the request.

• The physician determines the requested documentation cannot be completed.

 

Step 6

Completed documentation is securely delivered through the patient portal.

Most requests range between $350 and $495 CAD.

 

If additional physician time is required beyond the initial estimate, this will be discussed with you before any further work is undertaken.
