Where sufficient clinical information is available, your insurance form may be completed without an appointment. If additional information or a medical consultation is required, we'll explain the next steps before proceeding.

What You'll Need to Provide

After payment, you'll receive access to our secure patient portal where you may be asked to upload:

  • Government-issued photo identification

  • Insurance company's medical form

  • Insurance correspondence outlining the request

  • Relevant medical records

  • Specialist consultation reports (if applicable)

  • Current medication list

  • Hospital discharge summaries (if applicable)

  • Signed patient consent

Additional documentation may be requested depending on the complexity of your claim.

How It Works

 

Step 1

Select Standard Insurance Form and complete payment.

 

Step 2

Our staff will create your secure patient account.

You'll receive an email invitation to activate your secure patient portal.

 

Step 3

Log into the secure patient portal to:

  • Complete your intake questionnaire

  • Sign the required consent forms

  • Upload your insurance forms

  • Upload supporting medical documentation

 

Step 4

A licensed physician reviews:

  • your insurance documentation;

  • your supporting medical records;

  • specialist reports (where applicable); and

  • the clinical information available.

Following review, the physician will determine whether sufficient information exists to complete the requested insurance documentation.

 

Step 5

One of the following outcomes will occur:

  • Your insurance form is completed.

  • Additional documentation is requested.

  • A medical consultation or examination is required.

  • The physician advises that a more comprehensive service is required.

  • The physician determines the requested documentation cannot be completed.

 

Step 6

If completed, your insurance documentation will be securely delivered through the patient portal.

 

Fees

This fee includes physician review of your documentation and completion of one standard insurance form where clinically appropriate.

Should additional consultations, examinations, extensive record review, or completion of multiple forms be required, any additional fees will be discussed with you before proceeding.

 

Important Information

Insurance documentation often requires physicians to certify medical conditions, functional limitations, treatment history, prognosis, and other clinical information.

Our physicians are professionally and ethically responsible for ensuring that every insurance form accurately reflects the available clinical information and complies with accepted medical standards.

Following review, the physician may determine that:

  • the insurance form can be completed;

  • additional medical records are required;

  • specialist reports are needed;

  • a medical consultation or examination is necessary;

  • the request requires a more comprehensive insurance review package; or

  • the requested insurance documentation cannot be completed because the available clinical information does not support it.

 

Submission of a request does not guarantee that an insurance form will be completed or that any insurance claim will be approved.

Insurance coverage decisions are made solely by the insurance provider.
