What You'll Need to Provide

After payment, you'll receive access to our secure patient portal where you may be asked to provide:

If a medical consultation or examination is required before the certificate can be completed, you will be advised before any additional services are provided.

Important Information

Our coordinated physicians are professionally and ethically responsible for ensuring that every medical certificate accurately reflects the available clinical information and complies with accepted medical standards.

Following review, the physician may determine that:

the medical certificate can be completed;
additional supporting documentation is required;
a medical consultation or physical examination is necessary before the certificate can be completed; or
the requested medical certificate cannot be completed because there is insufficient clinical information or the request cannot be medically supported.

Submission of a request does not guarantee that a physician will complete or sign the requested medical certificate.

Physicians may only complete medical documentation when they are satisfied that it is clinically appropriate to do so.
