Following review, one of the following may occur:

Your sick note is completed.
Additional documentation is requested.
A medical consultation is required.
The physician determines that a sick note cannot be issued.

Step 5

If completed, your doctor's sick note will be securely delivered through the patient portal.

Fees

This fee includes physician review of your request and completion of the sick note where clinically appropriate.

If a medical consultation or examination is required before the physician can complete your request, you will be advised before any additional services are provided.

Important Information

Our physicians are professionally and ethically responsible for ensuring that all medical documentation accurately reflects the available clinical information.

A physician may determine that:

additional documentation is required;
a medical consultation is necessary before a sick note can be completed; or
a sick note cannot be issued based on the information available.

Payment of the Professional Review & Completion Fee does not guarantee that a doctor's sick note will be issued.

Professional Review & Completion Fee

The Professional Review & Completion Fee covers administrative processing, secure patient portal setup, and physician review of your request. Once your request has been submitted and physician review has commenced, this fee is non-refundable, regardless of whether:

additional documentation is required;
a medical consultation or examination is required before the form can be completed; or
the physician determines that the requested form cannot be completed based on the information available.

If a medical consultation or examination is required, any additional fees or billing arrangements will be explained before proceeding. Exception: If PEIDoctor.com is unable to assign your request to a physician for review, the Professional Review & Completion Fee will be refunded in full.

Can I receive a sick note without an appointment?

Sometimes. If the physician has sufficient information to complete your request safely and professionally, an appointment may not be required.

What if I need a consultation?

If the physician determines that additional assessment is required, we'll contact you through the secure patient portal with the next steps.

How long does the review take?

Most requests are reviewed within the timeframe indicated at the time of purchase. Complex requests or those requiring additional information may take longer.

Are my documents secure?
